Medicine’s services.<\/strong> By accessing our website, completing our intake forms, or enrolling in a membership, you agree to be bound by these terms. If you do not agree, do not use our services.<\/p>\n<\/div>\n\n<h2>1. About FRAME Medicine<\/h2>\n<p>FRAME Medicine is a physician-directed men’s health telemedicine practice operating from Richmond Hill, Georgia. Medical services are provided by or under the supervision of Dr. Colin Sheffield, DO, a board-certified physician licensed in the states in which FRAME operates. FRAME Medicine is co-founded by Tom Rubenstein (CFO) and Dr. Colin Sheffield, DO (Medical Director).<\/p>\n<p><strong>Currently licensed states:<\/strong> Florida, Georgia, Washington, Nebraska, and North Carolina. Coaching services are available nationwide.<\/p>\n\n<h2>2. Eligibility<\/h2>\n<p>To use FRAME Medicine’s medical services, you must:<\/p>\n<ul>\n <li>Be 18 years of age or older<\/li>\n <li>Be a resident of a state in which FRAME Medicine is licensed<\/li>\n <li>Provide accurate and complete health information during intake<\/li>\n <li>Not have any contraindication to the treatment you are requesting, as determined by physician review<\/li>\n<\/ul>\n<p>Coaching services (provided by Tom Rubenstein) are available to individuals 18 and older in all 50 states without a physician requirement.<\/p>\n\n<h2>3. Not an Emergency Service<\/h2>\n<p><strong>FRAME Medicine is not an emergency medical service.<\/strong> If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Telemedicine is not a substitute for emergency care, urgent care, or primary care for acute conditions.<\/p>\n\n<h2>4. Telehealth Informed Consent<\/h2>\n<p>By using FRAME Medicine’s telemedicine services, you acknowledge and consent to the following:<\/p>\n<ul>\n <li>Telemedicine involves the delivery of healthcare services using electronic communications and does not involve in-person physical examination<\/li>\n <li>The physician may determine that your condition requires in-person evaluation or referral to a specialist<\/li>\n <li>Telehealth services carry inherent limitations compared to in-person care, including limited ability to perform physical examination<\/li>\n <li>You have the right to withhold or withdraw consent at any time without affecting your right to future care<\/li>\n <li>All telemedicine sessions and records are subject to HIPAA privacy protections as described in our Privacy Policy<\/li>\n<\/ul>\n\n<h2>5. Membership Plans and Pricing<\/h2>\n<p>FRAME Medicine offers the following membership plans. This covers your first lab panel and physician review and is billed separately from your monthly membership. Lab fees for follow-up bloodwork are billed separately at clinically indicated intervals.<\/p>\n\n<h2>6. Billing and Payment<\/h2>\n<ul>\n <li>Membership fees are billed monthly to your payment method on file<\/li>\n <li>Payment is due at the start of each billing cycle<\/li>\n <li>Failure to maintain a valid payment method may result in service interruption<\/li>\n <li>We do not store full credit card numbers; payment processing is handled by a PCI-compliant processor<\/li>\n <li>All charges are in USD<\/li>\n<\/ul>\n\n<h2>7. Cancellation Policy<\/h2>\n<p>Coaching memberships may be cancelled at any time with no penalty by contacting FRAME Medicine.<\/p>\n<p>Medical memberships (TRT, GLP-1, combination plans) have minimum term commitments as listed in Section 5. Early cancellation of a medical membership is subject to the following:<\/p>\n<ul>\n <li>You must notify FRAME Medicine in writing at least 5 business days before your next billing date<\/li>\n <li>If you are on a protocol that requires medical taper or titration to safely discontinue, Dr. Sheffield will advise you on a safe discontinuation plan before your membership ends<\/li>\n <li>FRAME Medicine reserves the right to decline cancellation requests where doing so would pose a medical risk to the patient, pending physician guidance<\/li>\n<\/ul>\n\n<h2>8. Prescription and Medication Policy<\/h2>\n<p>All prescriptions are issued at the sole discretion of Dr. Sheffield based on clinical evaluation, lab results, and applicable medical standards. Requesting a service does not guarantee a prescription will be issued. FRAME Medicine uses licensed compounding pharmacies; compounded medications are not FDA-approved as finished drug products.<\/p>\n<p>You agree to:<\/p>\n<ul>\n <li>Use medications only as prescribed and directed<\/li>\n <li>Notify Dr. Sheffield of any changes in your health status, medications, or adverse reactions<\/li>\n <li>Complete required follow-up labs at the intervals specified by Dr. Sheffield<\/li>\n <li>Not share, sell, or distribute prescribed medications<\/li>\n<\/ul>\n\n<h2>9. SMS Communications and CTIA Compliance<\/h2>\n<p>FRAME Medicine uses Twilio to send SMS communications related to your care. By providing your mobile number and consenting during intake, you agree to receive text messages from FRAME Medicine at +1 (239) 372-6339, including:<\/p>\n<ul>\n <li>Appointment and check-in reminders<\/li>\n <li>Prescription refill notifications<\/li>\n <li>Lab result alerts<\/li>\n <li>Care-related follow-up messages<\/li>\n<\/ul>\n<p><strong>Message frequency:<\/strong> Varies by care plan (typically 2-6 per month). <strong>Message and data rates may apply.<\/strong><\/p>\n<p>Reply <strong>STOP<\/strong> to opt out. Reply <strong>HELP<\/strong> for assistance. Opting out of SMS will not affect your medical care. Your phone number is never shared with third parties for marketing.<\/p>\n<p>Our SMS program is registered under A2P 10DLC and complies with CTIA Messaging Principles and Best Practices.<\/p>\n\n<h2>10. Coaching Services Disclaimer<\/h2>\n<p>Coaching services provided by Tom Rubenstein are not medical services and do not create a physician-patient relationship. Coaching includes training program design, nutritional guidance, and lifestyle recommendations based on your self-reported health status and goals. Coaching is not a substitute for medical advice, diagnosis, or treatment. Always consult a qualified physician before starting a new exercise or nutrition program.<\/p>\n\n<h2>11. No Guarantees of Medical Outcomes<\/h2>\n<p>FRAME Medicine does not guarantee specific medical outcomes. Individual results from any treatment \u2014 including testosterone therapy, GLP-1 medications, ED treatment, or hair loss therapy \u2014 vary based on individual physiology, adherence, and other factors. Testimonials and statistics shared on our website reflect general clinical data and individual patient experiences; they are not predictions of your specific results.<\/p>\n\n<h2>12. Prohibited Uses<\/h2>\n<p>You agree not to:<\/p>\n<ul>\n <li>Provide false or misleading information during intake or at any time during treatment<\/li>\n <li>Use FRAME Medicine’s services to obtain controlled substances for any purpose other than personal therapeutic use as prescribed<\/li>\n <li>Attempt to circumvent physician review or clinical safeguards<\/li>\n <li>Access another patient’s account or health information<\/li>\n <li>Use our website or services in any manner that violates applicable law<\/li>\n<\/ul>\n\n<h2>13. Limitation of Liability<\/h2>\n<p>TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, FRAME MEDICINE, ITS PHYSICIANS, OFFICERS, AND STAFF SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF OUR SERVICES. OUR TOTAL LIABILITY TO YOU FOR ANY CLAIM ARISING FROM THESE TERMS SHALL NOT EXCEED THE TOTAL AMOUNT YOU PAID TO FRAME MEDICINE IN THE THREE (3) MONTHS PRECEDING THE CLAIM.<\/p>\n\n<h2>14. Indemnification<\/h2>\n<p>You agree to indemnify and hold harmless FRAME Medicine, its co-founders, physicians, officers, and staff from any claims, damages, or expenses (including reasonable attorneys’ fees) arising out of your breach of these Terms, your misuse of our services, or your provision of false or misleading health information.<\/p>\n\n<h2>15. Governing Law and Dispute Resolution<\/h2>\n<p>These Terms are governed by the laws of the State of Georgia. Any disputes arising from these Terms or your use of FRAME Medicine’s services shall be resolved in the courts of Bryan County, Georgia. You agree to personal jurisdiction in this venue.<\/p>\n\n<h2>16. Changes to These Terms<\/h2>\n<p>FRAME Medicine reserves the right to update these Terms of Service at any time. Material changes will be communicated via email or website notice. Continued use of our services after changes are posted constitutes acceptance of the updated Terms.<\/p>\n\n<h2>17. Medicine’s services.<\/strong> By accessing our website, completing our intake forms, or enrolling in a membership, you agree to be bound by these terms. If you do not agree, do not use our services.<\/p>\n<\/div>\n\n<h2>1. About FRAME Medicine<\/h2>\n<p>FRAME Medicine is a physician-directed men’s health telemedicine practice operating from Richmond Hill, Georgia. Medical services are provided by or under the supervision of Dr. Colin Sheffield, DO, a board-certified physician licensed in the states in which FRAME operates. FRAME Medicine is co-founded by Tom Rubenstein (CFO) and Dr. Colin Sheffield, DO (Medical Director).<\/p>\n<p><strong>Currently licensed states:<\/strong> Florida, Georgia, Washington, Nebraska, and North Carolina. Coaching services are available nationwide.<\/p>\n\n<h2>2. Eligibility<\/h2>\n<p>To use FRAME Medicine’s medical services, you must:<\/p>\n<ul>\n <li>Be 18 years of age or older<\/li>\n <li>Be a resident of a state in which FRAME Medicine is licensed<\/li>\n <li>Provide accurate and complete health information during intake<\/li>\n <li>Not have any contraindication to the treatment you are requesting, as determined by physician review<\/li>\n<\/ul>\n<p>Coaching services (provided by Tom Rubenstein) are available to individuals 18 and older in all 50 states without a physician requirement.<\/p>\n\n<h2>3. Not an Emergency Service<\/h2>\n<p><strong>FRAME Medicine is not an emergency medical service.<\/strong> If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Telemedicine is not a substitute for emergency care, urgent care, or primary care for acute conditions.<\/p>\n\n<h2>4. Telehealth Informed Consent<\/h2>\n<p>By using FRAME Medicine’s telemedicine services, you acknowledge and consent to the following:<\/p>\n<ul>\n <li>Telemedicine involves the delivery of healthcare services using electronic communications and does not involve in-person physical examination<\/li>\n <li>The physician may determine that your condition requires in-person evaluation or referral to a specialist<\/li>\n <li>Telehealth services carry inherent limitations compared to in-person care, including limited ability to perform physical examination<\/li>\n <li>You have the right to withhold or withdraw consent at any time without affecting your right to future care<\/li>\n <li>All telemedicine sessions and records are subject to HIPAA privacy protections as described in our Privacy Policy<\/li>\n<\/ul>\n\n<h2>5. Membership Plans and Pricing<\/h2>\n<p>FRAME Medicine offers the following membership plans. This covers your first lab panel and physician review and is billed separately from your monthly membership. Lab fees for follow-up bloodwork are billed separately at clinically indicated intervals.<\/p>\n\n<h2>6. Billing and Payment<\/h2>\n<ul>\n <li>Membership fees are billed monthly to your payment method on file<\/li>\n <li>Payment is due at the start of each billing cycle<\/li>\n <li>Failure to maintain a valid payment method may result in service interruption<\/li>\n <li>We do not store full credit card numbers; payment processing is handled by a PCI-compliant processor<\/li>\n <li>All charges are in USD<\/li>\n<\/ul>\n\n<h2>7. Cancellation Policy<\/h2>\n<p>Coaching memberships may be cancelled at any time with no penalty by contacting FRAME Medicine.<\/p>\n<p>Medical memberships (TRT, GLP-1, combination plans) have minimum term commitments as listed in Section 5. Early cancellation of a medical membership is subject to the following:<\/p>\n<ul>\n <li>You must notify FRAME Medicine in writing at least 5 business days before your next billing date<\/li>\n <li>If you are on a protocol that requires medical taper or titration to safely discontinue, Dr. Sheffield will advise you on a safe discontinuation plan before your membership ends<\/li>\n <li>FRAME Medicine reserves the right to decline cancellation requests where doing so would pose a medical risk to the patient, pending physician guidance<\/li>\n<\/ul>\n\n<h2>8. Prescription and Medication Policy<\/h2>\n<p>All prescriptions are issued at the sole discretion of Dr. Sheffield based on clinical evaluation, lab results, and applicable medical standards. Requesting a service does not guarantee a prescription will be issued. FRAME Medicine uses licensed compounding pharmacies; compounded medications are not FDA-approved as finished drug products.<\/p>\n<p>You agree to:<\/p>\n<ul>\n <li>Use medications only as prescribed and directed<\/li>\n <li>Notify Dr. Sheffield of any changes in your health status, medications, or adverse reactions<\/li>\n <li>Complete required follow-up labs at the intervals specified by Dr. Sheffield<\/li>\n <li>Not share, sell, or distribute prescribed medications<\/li>\n<\/ul>\n\n<h2>9. SMS Communications and CTIA Compliance<\/h2>\n<p>FRAME Medicine uses Twilio to send SMS communications related to your care. By providing your mobile number and consenting during intake, you agree to receive text messages from FRAME Medicine at +1 (239) 372-6339, including:<\/p>\n<ul>\n <li>Appointment and check-in reminders<\/li>\n <li>Prescription refill notifications<\/li>\n <li>Lab result alerts<\/li>\n <li>Care-related follow-up messages<\/li>\n<\/ul>\n<p><strong>Message frequency:<\/strong> Varies by care plan (typically 2-6 per month). <strong>Message and data rates may apply.<\/strong><\/p>\n<p>Reply <strong>STOP<\/strong> to opt out. Reply <strong>HELP<\/strong> for assistance. Opting out of SMS will not affect your medical care. Your phone number is never shared with third parties for marketing.<\/p>\n<p>Our SMS program is registered under A2P 10DLC and complies with CTIA Messaging Principles and Best Practices.<\/p>\n\n<h2>10. Coaching Services Disclaimer<\/h2>\n<p>Coaching services provided by Tom Rubenstein are not medical services and do not create a physician-patient relationship. Coaching includes training program design, nutritional guidance, and lifestyle recommendations based on your self-reported health status and goals. Coaching is not a substitute for medical advice, diagnosis, or treatment. Always consult a qualified physician before starting a new exercise or nutrition program.<\/p>\n\n<h2>11. No Guarantees of Medical Outcomes<\/h2>\n<p>FRAME Medicine does not guarantee specific medical outcomes. Individual results from any treatment \u2014 including testosterone therapy, GLP-1 medications, ED treatment, or hair loss therapy \u2014 vary based on individual physiology, adherence, and other factors. Testimonials and statistics shared on our website reflect general clinical data and individual patient experiences; they are not predictions of your specific results.<\/p>\n\n<h2>12. Prohibited Uses<\/h2>\n<p>You agree not to:<\/p>\n<ul>\n <li>Provide false or misleading information during intake or at any time during treatment<\/li>\n <li>Use FRAME Medicine’s services to obtain controlled substances for any purpose other than personal therapeutic use as prescribed<\/li>\n <li>Attempt to circumvent physician review or clinical safeguards<\/li>\n <li>Access another patient’s account or health information<\/li>\n <li>Use our website or services in any manner that violates applicable law<\/li>\n<\/ul>\n\n<h2>13. Limitation of Liability<\/h2>\n<p>TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, FRAME MEDICINE, ITS PHYSICIANS, OFFICERS, AND STAFF SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF OUR SERVICES. OUR TOTAL LIABILITY TO YOU FOR ANY CLAIM ARISING FROM THESE TERMS SHALL NOT EXCEED THE TOTAL AMOUNT YOU PAID TO FRAME MEDICINE IN THE THREE (3) MONTHS PRECEDING THE CLAIM.<\/p>\n\n<h2>14. Indemnification<\/h2>\n<p>You agree to indemnify and hold harmless FRAME Medicine, its co-founders, physicians, officers, and staff from any claims, damages, or expenses (including reasonable attorneys’ fees) arising out of your breach of these Terms, your misuse of our services, or your provision of false or misleading health information.<\/p>\n\n<h2>15. Governing Law and Dispute Resolution<\/h2>\n<p>These Terms are governed by the laws of the State of Georgia. Any disputes arising from these Terms or your use of FRAME Medicine’s services shall be resolved in the courts of Bryan County, Georgia. You agree to personal jurisdiction in this venue.<\/p>\n\n<h2>16. Changes to These Terms<\/h2>\n<p>FRAME Medicine reserves the right to update these Terms of Service at any time. Material changes will be communicated via email or website notice. Continued use of our services after changes are posted constitutes acceptance of the updated Terms.<\/p>\n\n<h2>17.
